This waiver applies to participation in ELEVATE Golf Academy camps, clinics, intensives, and training programs. Agreement to this waiver is completed electronically during registration.
Program Overview
ELEVATE Golf Academy (“ELEVATE”) offers youth golf camps and training programs that may take place at Metropolitan Golf Links and other approved facilities.
Participation may include, but is not limited to:
Golf instruction and training activities
Outdoor physical activity
Use of golf equipment and practice facilities
Medical Information & Disclosure
Parents or legal guardians are responsible for disclosing all relevant medical information during registration, including but not limited to:
Medical conditions or diagnoses
Physical limitations or accommodations
Allergies or dietary restrictions
Medications that may be required during camp
This information is collected through the registration form and reviewed by ELEVATE staff prior to participation.
Medical Treatment Authorization
In the event of illness or injury, I authorize ELEVATE Golf Academy staff to obtain necessary medical treatment for my child, including emergency care if required.
I understand and agree that I am financially responsible for any medical services provided to my child as a result of participation in ELEVATE programs.
Liability Waiver & Release
I acknowledge that participation in ELEVATE Golf Academy programs involves inherent risks, including but not limited to physical injury, illness, or other unforeseen hazards.
On behalf of myself and my child, I voluntarily assume all such risks and hereby release and hold harmless ELEVATE Golf Academy, LLC, its owners, officers, employees, contractors, volunteers, host facilities (including Metropolitan Golf Links), and affiliated partners from any and all claims, liabilities, or damages arising from participation, except in cases of gross negligence or willful misconduct.
I affirm that my child is physically capable of participating and agrees to follow all rules, instructions, and safety guidelines provided by ELEVATE staff. I understand that dismissal from a program for misconduct or failure to comply with rules does not entitle me to a refund.
Photo & Video Release
I grant permission for ELEVATE Golf Academy to use photographs and/or video recordings of my child for educational, marketing, and promotional purposes, including use on websites, social media, and printed materials, without compensation.

Electronic Acceptance Notice
This waiver is accepted electronically during the registration process through a required acknowledgment checkbox and parent/guardian digital signature. Submission details, including timestamp and IP address, are recorded for verification.
